A HAZOP (Hazard and Operability study) is a structured, team-based review that examines a process design node by node, applying guide words no flow, more pressure, reverse, high temperature to each P&ID against its design intent. For every credible deviation, the team works through the cause, the consequence, the existing safeguards, and any recommendation needed. It’s one of the most widely required hazard identification techniques in high-hazard industry & the primary input to LOPA & SIL determination.
A HAZOP is only as good as the engineer chairing it. Run it in-house under deadline pressure & get the predictable failure modes: nodes rushed, dominant voices unchallenged, deviations missed, and a recommendations register no one ever closes. On paper you’ve done a HAZOP. In reality you’ve created a documented record of hazards you knew about and didn’t act on the single most damaging exhibit in any incident investigation or DOSH enquiry.
An independent chair changes the economics. A missed high-consequence deviation found in a workshop costs a recommendation. The same deviation found after start-up costs a shutdown, a repair, lost production & potentially a major accident. With Malaysian enforcement tightening since the OSHA 1994 Amendment took effect in June 2024, a defensible, fully closed-out HAZOP isn’t a compliance chore; it’s one of the cheapest risk reductions available to you.
